St. Joseph Preschool has begun Registration for the 2010-2011 school year.  Registration forms are available on-line by clicking on the links below or copies may be picked up in the parish office during work hours.

Registration forms and fees will be accepted on the following schedule:

Current Students: January 25th
Parish Members: February 8th
General Public: February 22nd

Registration forms may be picked up or downloaded at any point, but will not be accepted before the above mentioned dates.  Registration forms must be turned in to the parish office (phone calls, e-mails, and letters will not be accepted).  A non-refundable Registration Fee of $35 is due at the time the Registration Form is turned in.

Our physical examination and immunization requirements are also available on-line or in the parish office.  As a DCFS certified institution, we use the DCFS Certificate of Child Health Examination Form which is linked below.  It must be filled out by your doctor and returned prior to the start of the school year.
